Application and Procedures for Use of School Facility
  • Please complete this form to request the use of school facilities.

  • The completed form should be submitted to building principal.

  • This application must be approved before a group is permitted to use school facilities.

  • Please check the Calendar on our website for availability of the facility you are requesting to use.
  • Usage fees are as follows:
    • Elementary Gym - $10.00
    • High School Gym - $15.00
    • Community Room - No Fee
  • A key deposit of $25.00 will be required when keys are picked up. The deposit will be returned when keys are turned in.
